To use our services, you must meet the following criteria:
- Must be located in California.
- Must be 18 years of age or older.
- Must have been diagnosed with an illness known to be treatable or
alleviated through the use of therapeutic cannabis. - Must have a recommendation from a Doctor. You are strongly advised to have a state-issued (Department of Health) DHS card for maximum protection under the law.
You must complete our Membership Registration Form. You can fax this and the accompanying documents to our office prior to delivery or hand it to the driver upon delivery. We will verify your information which enables us to operate Plant Providers Plus legally according to Prop 215 and Health & Safety Code Sec. 11362.5(B)(1)(A) & 11362.7(H).
PATIENTS MUST HAVE A RECOMMENDATION FROM A LICENSED DOCTOR, HAVE THEIR ORIGINAL RECOMMENDATION LETTER AND A VALID CA I.D. ALL DOCTORS RECOMMENDATIONS WILL BE VERIFIED.
Privacy Policy
We ask for and verify your personal information which enables us to operate Plant Providers as required by law. We do not give away or sell your personal information. We simply verify your information which allows Plant Providers Plus to operate legally according to Prop 215 and Health & Safety Code Sec. 11362.5(B)(1)(A) & 11362.7(H). Your personal information is maintained by on-site secured computers as well as an off-site secured server.
