In order to join our Collective and receive member benefits and discounts for medicinal marijuana, please complete the Membership Registration Form (PDF).
If you are a first time patient, please complete this form, scan it and email it to our office, along with your CA Driver’s License and Doctor’s Authorization. Or, if you don’t have scan capabilities, you may fax your documents.

If all of your information is valid and up to date, we will then verify and approve you for delivery.
Upon all deliveries, you must present your valid Medical Marijuana ID Card to the driver.
