If you are a first time patient, please complete this form, scan it and email it to our office, along with your CA Driver’s License and Physician’s Recommendation. Or, if you don’t have scanning capabilities, you may fax your documents. Fax number is provided on the bottom of the registration form.
If you unable to provide your documents ahead of time, we can verify you prior to delivery. Then you must present your original, valid physician’s recommendation to the driver upon delivery.

1. Simply click on the Membership Registration Form and complete the Membership Form. You must also provide your physician’s recommendation and California driver’s license.
2. Select your medicine from the Menu page.
3. Call (408) 824-1184 to place your order and schedule a delivery.
4. First time patients of medicine receive a free Medible (sorry, does not include clone orders).
You must present your valid Medical Marijuana physician’s recommendation to the driver each time a delivery is received.
